ILIT is a newer approach to allergy immunotherapy where tiny doses of allergen are injected directly into a lymph node rather than under the skin. Because lymph nodes are where the immune system learns and adapts, this targeted delivery may help your body build tolerance faster — with far fewer injections than traditional allergy shots. This handout explains what ILIT is, who it's for, what to expect, and what it costs.

1 What Is ILIT?

ILIT stands for intralymphatic immunotherapy. Like traditional allergy shots, it works by exposing your immune system to small amounts of the things you're allergic to — but the method is different. Rather than injecting allergen under the skin, a provider uses ultrasound guidance to deliver the dose directly into a lymph node in the groin area.

💡 Why the Lymph Node?

Lymph nodes are the immune system's "training centers." Delivering allergen there may help your immune system become tolerant more efficiently — potentially achieving in 3 visits what traditional shots work toward over several years.

3 Before You Start

Before beginning ILIT, your allergist will take several steps to make sure the treatment is right for you.

  2. 2Confirm your allergies with skin testing or blood tests
  3. 3Decide which allergens to include (e.g., specific pollens, dust mites, or pet dander)
  4. 4Discuss benefits, risks, and alternatives — including traditional shots, sublingual tablets, or medications alone

4 What Happens on Treatment Days

Each visit follows the same routine. Plan to be at the clinic for approximately 45–75 minutes including your observation period.

  2. 2The skin over the groin/lower abdomen area is cleaned and prepared
  3. 3Ultrasound is used to locate the lymph node precisely before the injection
  4. 4A small needle delivers a tiny amount of allergen — the injection itself takes only a few seconds
  5. 5You remain in the office for 30–60 minutes of observation so staff can watch for any reactions and treat them immediately if needed
Plan ahead: You must stay for the full observation period after each injection. Please arrange your schedule accordingly.

5 What Results Can You Expect?

Results vary between patients, but many people with pollen and dust mite allergies report meaningful improvement. Clinical studies — particularly in patients with grass and birch pollen allergies — found that ILIT improved symptoms and quality of life, with benefits lasting into subsequent allergy seasons after just the short treatment series.

  • This is a newer treatment — not all providers offer it

6 Side Effects and Risks

Most side effects are mild and temporary. As with all forms of allergy immunotherapy, serious reactions are rare but possible — this is why ILIT is always done under medical supervision.

Common & Mild

  • Soreness, pressure, or bruising at the injection site
  • Mild swelling or tenderness in the groin area
  • Temporary increase in allergy symptoms (e.g., congestion or sneezing)

Less Common & Serious

  • Hives, wheezing, or difficulty breathing
  • Anaphylaxis — a severe, potentially life-threatening allergic reaction (rare)
⚠️ After you leave the clinic: If you develop trouble breathing, swelling of the lips or throat, dizziness, or widespread hives — call 911 or go to the nearest emergency room immediately. Do not wait to see if symptoms improve on their own.

9 Insurance & Payment Options

Most insurance plans currently do not cover the ILIT procedure itself, as it is considered an off-label use of existing allergy extracts. However, related services — office visits and allergy testing — are typically billed to your insurance in the usual way.
